I get randomly strong short pain "strikes" in my upper back / neck area. I work with computer 5 days a week, 8 hours per day. Could physiotherapy help?
Physiotherapy is highly effective for this. Sharp, intermittent pain strikes during prolonged desk work typically stem from cumulative load, sustained postural stress, and localized cervicothoracic junction stiffness rather than structural damage. Physiotherapy resolves these episodes through active load management, restoring segmental thoracic and cervical mobility, and retraining deep cervical flexors and scapular stabilizers to improve load distribution. An individualized clinical evaluation can pinpoint whether your episodes relate to localized facet joint irritation, myofascial trigger points, or postural fatigue, tailoring an active pro...gram to eliminate the strikes. Yes, physiotherapy can help a lot with this, especially when you spend many hours at a computer.
Very often, this pattern is related to trigger points, deep hypersensitive tight spots in muscles (upper trapezius, levator scapulae, suboccipitals, rhomboids) that don’t show up in scans, but can create sharp, sudden pain and referred symptoms. If they’re not released, they can keep the symptom active even if nothing “serious” is shown in scans.
What physiotherapy would focus on
- Trigger point therapy with sustained pressure (ischemic compression) to release the tight spots and reduce the symptoms.
- Posture and worksta...tion-related load management (how your neck/shoulders are being overloaded during the day).
- Simple strengthening for scapular control and upper back endurance, so the problem doesn’t keep coming back.
One quick question that helps us guide you:
Does heat or cold relieve it more? If heat helps, it often suggests muscle tension and trigger points are a big part of the picture and worth targeting.

Yes, physiotherapy may help, particularly if the symptoms are related to prolonged static posture, muscle overload or reduced neck and upper-back mobility. An assessment can identify contributing factors and guide a gradual programme of mobility, strengthening, workstation adjustments and regular movement breaks.
However, because you describe strong, sudden pain “strikes”, it would be advisable to have an assessment rather than assume that posture is the only cause. Seek prompt medical advice if the pain is accompanied by chest pain, shortness of breath, fever, recent trauma, weakness or numbness in an arm, balance problems, a severe... headache or persistent night pain.
