Η εκτίμηση του χειρουργικού κινδύνου δεν γίνεται έτσι απλά. Εξαρτάται από την βαρύτητα της καρδιακής ανεπάρκειας αλλά και την γενικότερη κατάσταση του ασθενή (αναπνευστικό, παχυσαρκία άλλες παθήσεις όπως σάκχαρο, ηυρεοειδής κλπ) αλλά και από την άλλη μεριά από την αναγκαιότητα της επέμβασης. Σε βαρύτατους ασθενείς υπάρχουν άλλες επεμβάσεις που γίνονται αντί για την χολοκυστεκτομή.

Εξαρτάται από το είδος της ανεπάρκειας. Συνήθως η λαπαροσκοπική χολοκυστεκτομή είναι εφικτή χωρίς πρόβλημα άλλα θα πρέπει να δούμε ή να οργανώσουμε ένα υπερηχογράφημα καρδίας για να διευκρινήσουμε εάν υπάρχει κίνδυνος ή όχι.
